Once you've enabled and created failure codes, you can begin configuring a failure hierarchy for assets or asset categories to associate specific problems, causes, and actions with each other.
For example, the Vehicle asset category could have a number of problems, including running rough. This problem could be associated with specific causes (a sticky valve) and actions (replacing the valve). If you were to select running rough as the problem on the work order form after creating this hierarchy, the options for causes and problems would only include those associated with this problem.
To configure a failure hierarchy:
- Navigate to Settings > CMMS Settings.
- Click Maintenance and Work Orders.
- Under Failure Codes, select the Enable strict problem, cause, action nesting for failure codes checkbox:

Selecting this checkbox restricts users from selecting problems, causes, and actions that are not included in asset or asset category failure hierarchies. It also limits options according to the selected hierarchy (only causes and actions associated with the selected problem will appear).
- Under Failure Code Nesting Configuration, click either Configure Nesting for Asset Categories or Configure Nesting for Assets:
- Select the asset or asset category you want to configure a hierarchy for.
- Select the problems to associate with the asset or asset category:
To select more than one item, hold down the shift key when clicking. Selected items will display in deeper black. In the above image, the Vehicles category is selected, as are all problems except P5. - Click Save Asset Category or Save Asset Category Configuration, depending on whether you're configuring for an asset or asset category.
- Click the Causes and Actions tab:
- Select a problem and then one or more causes to associate it with:
In the above example, C1 has been associated with P1. - Select one or more actions to associate with the problem:
- Click Save Action Cause:
- Repeat steps 9-11 as needed for different problems.
- Click Save in the top menu bar.
Your failure hierarchy is now configured and will reflect in your work order forms.
